MyFHN is an online tool that helps you manage your healthcare easily and securely. To enroll, click on the Enroll button to the right; there are two options available for enrollment.
After you've received your medical records number and entered your information, you will receive an email with a one-time user ID and password, along with a link that will enable you to complete the enrollment process.
Your My FHN Patient Portal has just become even more convenient! You can now - in addition to calling your pharmacy - request to have your prescription renewed using the MyFHN Patient Portal. It's easy, just click on the Medications tab, then click on the medication you need refilled, and request renewal. This will notify your provider and they will message you back once the refill has been sent in.

With our new MyFHN app, you can easily access your healthcare information on your smartphone any time! You can choose to log in using the website too, but if you’d like to take advantage of functionality designed just for your phone or tablet, just follow these simple steps:
A Proxy Authorization Form is required to access the health information of anyone over age 12 by someone other than themselves. Please note that all signatures must be witnessed by an FHN staff member. If you have any questions about this process, please contact the MyFHN support line at 815-599-6907. To revoke proxy permission, a Proxy Revocation form must be completed.
