Freeport School District Sports Physicals

 

This year's sports physicals are currently being scheduled and will be added as soon as dates and times are confirmed.

Freeport's 2024-2025 sports physical clinics will be held at FHN Family Healthcare Clinic – Burchard Hills, 1010 W. Fairway Drive, Freeport.

The following forms must be completed and signed by a parent or guardian before arriving for the physical:

These exams meet Illinois eligibility requirements for all sports and are good through the 2024-25 school year. These exams also meet Wisconsin eligibility requirements.

These physicals do not fulfill the requirement for a full school physical. The State of Illinois requires school physicals for children entering kindergarten, sixth grade or transferring from another state. The state also requires dental exams for children entering kindergarten, second and sixth grades and vision exams for children entering kindergarten and transfer students. Contact your child's healthcare provider to schedule a school physical.

  • 5:00 - 5:30 pm — Football, Cross Country & Golf
  • 5:30 - 6:00 pm — Soccer & Dance
  • 5:30 - 6:00 pm — Phantoms & Wrestling
  • 6:00 - 6:30 pm — Volleyball, Tennis & Swimming
  • 6:30 - 7:00 pm — NJROTC & Softball

Tuesday, August 13, 5 pm - 7 pm

  • 5:30 - 6:00 pm — Basketball, Bowling & Cheer
  • 6:00 - 6:30 pm — Baseball, Track & Softball
  • 6:30 - 7:00 pm — Open for all sports

Any non-FHS athlete welcome at any time.
Designated times are to prevent long wait periods only.

Requirements

  • $25.00 upon registration via cash or check only. As in previous years, FHN donates this fee back to the school.
  • All forms (FHN consent and IHSA exam) filled out and signed prior to coming to the sports physical.
  • Only two adult support persons (one should be a parent/guardian) and athlete permitted in the building; no siblings or individuals not receiving a physical

Additional Information

  • Approximate exam time is 15 - 20 minutes.
  • Students will remain in one room for the exam.
  • Exam areas will be cleaned after each student.
  • Physical forms for all students will be turned over to the school at the end of each evening. Athletes can take a picture of the completed form with their phone for their records.
