FHN makes it easy for busy families to get middle- and high-school athletes' sports physicals with a series of $20 Sports Physical Clinics throughout the area. FHN donates all proceeds from these clinics to local schools.
The exams meet Illinois eligibility requirements for all sports and are good through the 2019 - 2020 school year. The IHSA/IEAA Sports Physical Form and the IHSA Sports Medicine Acknowledgement and Consent Form - Concussion, must be completed and signed by a parent or guardian before the physical. If a parent does not accompany the athlete, a parent or guardian must fill out and sign a Consent for Medical Care form. These physicals do not fulfill the requirement for a full school physical. The State of Illinois requires school physicals for children entering kindergarten, sixth grade or transferring from another state. The state also requires dental exams for children entering kindergarten, second and sixth grades and vision exams for children entering kindergarten and transfer students. Contact your child's healthcare provider to schedule a school physical.
