  • If this event is full, please continue to check this website, as well as FHN traditional and social media, for future dates which will be added as soon as possible. Schedules may change frequently based on availability of vaccines.
  • If you are unable to complete the registration form online, please call 815-599-6412 for assistance.
  • Please plan to stay for 15 minutes after you’ve received your vaccination or 30 minutes if you have had immediate reactions to vaccinations, such as flu shots, or need to regularly carry an EpiPen.
  • Information requested for identification is used only to confirm age and employment as it relates to groups receiving priority by the state of Illinois due to higher-risk exposure or potential for more serious COVID-19 infection; it is not needed, used, or retained for any other purpose.


  • Be patient. It will take time to vaccinate those in higher-risk groups before we can vaccinate the general public.
  • This is not a first-come, first-served process – there are many factors to consider when deciding when to vaccinate groups of people.
  • There is no specific timeline. We will proceed as fast as we can depending on vaccines and other resources as well as the number of people currently eligible for vaccination who need to be vaccinated first.
  • There is a limited supply of vaccine and resources.
  • This process may not be the same for other counties or locations.
