There is one Automated Teller Machine (ATM) located on the first floor near the Atrium (East) Entrance of the hospital located in the corridor between the Atrium and the Emergency Department.

Be a Good Healthcare Consumer
Healthcare is something we all invest in at one time or another, so it’s important to educate ourselves when it comes to consuming it. Just like you’d take the time to get the facts and investigate ratings when looking for a new car or other important investment, it’s equally important to make sure you are informed in order to make good choices when it comes to healthcare. There’s no more important time to be careful with your investments than during this uncertain economic era. So to make the most of your dollar and healthcare experience, here are a few tips that can help you become a better healthcare consumer.

Cafeteria and Vending Machines

Fresh Inspirations Café is located in the lower level of the hospital. The café offers sandwiches made on a panini grill press, healthy, delicious entrees and pizzas served from large heated pizza stones, as well as a full soup and salad bar, deli sandwiches and an extensive “grab and go” selection. Seattle’s Best coffee is available at a special gourmet coffee station, as well as cappuccino. Fresh Inspirations Café is open:

Monday through Friday
Breakfast: 7 – 10 a.m.
Lunch: 11 a.m. – 1:30 p.m.
Grilled items: 1:30 – 3:30 p.m.

Saturday and Sunday
Breakfast: 8 – 10:30 a.m.
Lunch: 11 a.m. – 2 p.m.

Holidays
11 a.m. – 2 p.m.

After hour alternatives:

  • Vending machines are available 24/7 in the lower level of the hospital next to the Fresh Inspirations Café.
  • Guests of patients may order an evening meal, until 6:30 p.m., using a credit or bank card for payment. It will be delivered to your patient’s room. Please ask nursing for an "At Your Request Room Service Dining Staff" form and Guest menu.

Chapel
Spiritual care provides a supportive and comforting role in the patient's healing process. Our commitment to spiritual care is exemplified through coordination of pastoral services with local churches and other communities of faith, respecting each patient/family's unique cultural, spiritual, and psychosocial needs. A chapel is located on the first floor of the hospital in the central corridor. Patients, family, and friends may use the chapel for prayer, reflection or private family meetings. The chaplain may be reached by calling 815-599-6160 or xt. 6160 from inside the hospital.

Complaint Resolution
If you have a customer service-related complaint, please speak to a staff member or contact FHN’s Customer Focus Consultant at 815-599-6356. Some of the customer service issues suitable for reporting to a staff member or the Customer Focus Consultant include:

  • Unacceptable wait times for appointments

  • Wrong meal tray served at the hospital

  • You didn’t feel respected by your caregiver or provider

  • Convenience and safety-related issues such as unshoveled sidewalks or spills

Each issue reported to the SilentWhistle and Customer Focus programs is brought to the attention of FHN administration and all other concerned parties. Your willingness to speak up about compliance or customer service issues at FHN helps us to continuously improve our care. Thank you for helping us maintain the highest standards of excellence in caring for our patients and our communities.

You may also send a complaint to the Secretary of Health & Human Services, 200 Independence Avenue, S.W., Washington, DC 20201, 1-800-HHS-TIPS (447-8477) and/or notify the Joint Commission Office of Quality Monitoring to report any concerns by calling 800-994-6610 or emailing complaint@jointcommission.org.

For mammography related complaints you may mail or fax the American College of Radiology, 1891 Preston White Drive, Reston, VA 20191-4397, fax (703) 648-9176, or email mamm-accred@acr.org. Follow-up will not occur with any complaint that is submitted verbally or anonymously.

Compliance
Health care organizations are governed by many federal, state and local laws and regulations. Knowing the rules and following the rules are both essential elements of health care compliance.

As part of its compliance effort, FHN has put in place a system which promotes the prevention, detection and resolution of instances of conduct that do not conform with the law or FHN policies.

Because the laws are very complex, sometimes an employee, patient or member of the public may not be sure whether a particular activity is in compliance with the law. In such cases, the organization has a compliance officer who is available to take reports and conduct investigations, a hotline phone number and a Web site for reporting. This is the means for reporting concerns or suspected violations of law or policy, not complaints.

If you feel you have been affected by or witnessed a questionable, unethical or illegal action at FHN, we urge you to report that incident to our anonymous hotline. We have contracted with an outside company to provide a SilentWhistle Web site and toll-free telephone number through which you can anonymously report any questionable actions. The SilentWhistle service is available to anyone who wishes to confidentially report business abuse and/or dishonesty. Some of the issues suitable for reporting to SilentWhistle include:

  • Coding or billing irregularities

  • Illegal activities on FHN premises

  • Questionable accounting

  • Falsification of information in medical records

  • HIPAA (patient privacy) violations

  • Conflict of interest

  • Substance abuse

Driving Directions and Parking Driving Directions
FHN Memorial Hospital is located in the heart of Freeport. From the north and south, come into Freeport on Illinois Highway 26. Turn west on Stephenson Street, proceed to the next set of stop lights.

Parking
FHN Memorial Hospital Parking is available in the north parking lot in front of the hospital accessed from Stephenson Street and in the area off of the Atrium (east) and Emergency Department entrances accessible from Whistler and Lincoln Streets. Parking is free at FHN Memorial Hospital and all FHN healthcare centers and offices. Please do not leave your vehicle in any restricted area.

FHN Health Resource Library
The FHN Health Resource Library believes that education is an important part of your healthcare. Access to high quality health information can help you make informed decisions about disease prevention, healthcare services, illness management, treatment options and follow-up care.

Who can use the library?
The FHN Health Resource Library is free and open to the public.

What does the library have to offer?

  • Free access to databases containing up-to-date healthcare information
  • Free printing and copying of health information
  • Ability to check out health resource materials from the collection after registering

Visitors are requested to use the computers primarily for health and wellness information searches.

FHN FastCare
Located inside the Freeport Shopko, FHN FastCare is open:
Monday – Friday 8:30 a.m. – 8:30 p.m.
Saturday 8:30 a.m. – 5 p.m.
Sunday 10 a.m. – 5 p.m.
Holidays 10 a.m. – 2 p.m.

You do not have to be an FHN patient to use FHN FastCare.

Patients must be 18 months or older.

For $52 a visit, FHN FastCare offers these services:

Gift Shop
The FHN Memorial Hospital Gift Shop offers mylar balloons, fresh and silk flower arrangements, fresh green plants and a variety of unique gift items. You will find a huge selection of greeting cards, baby gifts, food and candy items (even sugar-free), personal hygiene products and magazines, books, and other reading materials.

Special notice regarding latex balloons: Due to the increasing number of patients and staff with latex allergies, we ask that latex balloons not be sent or brought to the hospital. Mylar balloons are a good substitution.

Hearing Impaired Services and Foreign Language Interpreters
Services for the hearing impaired are available. Please ask a staff member to make arrangements for this service.

Foreign language interpreter services are available locally as well as through an AT&T interpreter service. Please ask a staff member if you or a family member need language assistance.

Illinois Poison Center
Illinois Poison Center logo
FHN acts as one of several satellite locations in Illinois for the Illinois Poison Center. The Illinois Poison Center serves all of Illinois 24 hours a day, 365 days a year. Staffed by nurses, physicians, pharmacists and other poison specialists, the IPC offers free, confidential poison prevention advice and treatment recommendations via a toll-free hot line, 1-800-222-1222.

Two information desks are conveniently located at FHN Memorial Hospital. The Main Desk is located in the Atrium, at the east entrance to the hospital. The second desk is located at the north entrance.

No Smoking Policy
FHN is committed to providing a healthy environment for all our patients, their family and visitors, and our staff members. As part of that commitment, we are a smoke-free campus. This includes all our facilities, parking lots and walkways.

Social Services
Social workers and other representatives from FHN’s Social Services department can assist patients and their families with the crises and emotional adjustments that frequently occur when a patient is ill or hospitalized. They can also help make arrangements for continuing care after the patient leaves the hospital. If you would like to see a social worker while you are a patient, please tell your nurse or call the department at 815-599-6149, 815-599-6139 or 815-599-6351.

Visitor Policy
FHN is committed to providing a safe, quiet and healthy environment for patients, their family and friends. When visiting your loved ones, please follow these rules:

  • Visiting hours are 8 a.m. to 8 p.m. Limiting the length of your visit will be helpful to your loved one.
    • All visitors must check in at the Visitor's Desk in the hospital atrium (east), just as you would check in when visiting a school or business.
    • Visitors will receive a Visitor's badge, which should be returned to the desk when leaving the hospital.
    • Young people under age 12 must remain on the first floor and lower level and be supervised by an adult at all times.
    • Each patient can have only 2 visitors at a time. This to minimize disturbances to other patients and ensure that caregivers have enough room at the bedside to provide care. Additional visitors can wait in the hospital atrium or designated waiting areas.
  • Please check with the nursing staff before bringing in food, personal items and flowers. Restricting these items may be an important part of patient care.
  • Some patients in our hospital are very ill. For their health, and yours, you may need to follow additional restrictions when visiting some areas, like Isolation. Also, please be considerate of roommates and other patients.
  • Informational signs are posted within the hospital and are meant to share important information. Please take the time to read them.
  • We are very respectful of our patients' privacy, so you may be asked to step out while treatments are being given.
  • Inappropriate behavior or refusing to follow hospital policy can compromise patient health and safety,and may result in visitors being asked to leave the premises.

Of course, no one should visit the hospital if they have cold or flu symptoms, such as a fever, cough, sore throat, runny nose or congestion, upset stomach or vomiting.

FHN Family Birthing Center Visiting Information
Please respect the safety and security needs of our smallest patients.

  • Visiting hours in the FHN Family Birthing Center are from 10:00 a.m. to 8:00 p.m.
  • All Family Birthing Center visitors must ring for entrance to the unit.
  • We ask that only four people visit at one time. There is a waiting room for additional visitors just outside the entrance to the FHN Family Birthing Center.
  • Siblings of the new baby are encouraged to visit as long as they are accompanied and monitored by an adult. For infection control purposes, other children under the age of 12 will not be permitted to visit. No children are allowed to spend the night in the FHN Family Birthing Center.
