FHN is offering a series of sports physical clinics at schools and FHN satellite locations across northwest Illinois. The physicals meet IHSA and WIAA eligibility requirements for sports, but do not meet the requirement for a ninth-grade school physical or for new out-of-state students.
The physicals are $20, with all proceeds going to area schools. Checks should be made out to FHN. A portion of the physical form must be completed and signed by a parent before the exam. If a parent does not accompany their student, a Consent for Medical Care form (available at your local FHN office or at the school office) must be signed and brought with the student. Downloadable, printable versions of the forms are available online.
No appointment is necessary unless it is noted below.
